SOCIAL WORKER/CASE MANAGER POSITION – OPEN

(2-POSITIONS)

Our COMPANY is seeking an EXPERIENCED SOCIAL WORKER/CASE MANAGER to direct patient care for our busy OPERATION located in NOVATO, CA. WE HAVE SEVERAL position OPENINGS full-time, 40 hours per week, part-time (16 hours) AND PER-DIEM AND SHORT/LONG TERM CONTRACTS ARE negotiable for the right candidate.

As a SOCIAL WORKER/CASE MANAGER you will be welcoming clients, coordinating their schedules, and assuring a warm experience for them throughout their INTERUCTIONS WITH PALLADIA HEALTH. We’re looking for a kind and friendly individual who is experienced in the medical field and passionate about the patient’s experience, coordinating care plans, and selling OUR SERVICES TO OUR FUTURE CLIENTS. We have a fantastic and loyal client base that we work with to achieve their HEALTHCARE GOALS. The ideal candidate not only strives to meet our clients’ needs but also enjoys taking a key role in HEALTH OPTIMIZATION tasks such as software utilization, inventory control, protocol implementation, and operational flow.

Job Description

Social Services Case Manager

We have case management positions available in our NOVATO, CALIFORNIA office.

Job Description: Are you passionate about providing support and caring for individuals OF ALL AGES, CULTURAL BACKGROUNDS AND IN NEED FOR HOME HEALTHJ SERVICES? Are you looking for an organization where we help our clients achieve their goals and dreams?

Job Responsibilities: Our Social Services Case Managers are responsible for using person-centered planning to assists clients in use of public and private resources to OBTAIN services.

Additional responsibilities of the Social Services Case Manager include:

  • Monitoring vendor services and verifying that services are delivered in accordance with the purchase of service authorizations
  • Evaluating the effectiveness of clients’ Individual Program Plans; ensuring that appropriate community resources are made available to the client
  • Conducting annual reassessments of clients’ PSYCOSOCIAL WELL-BEING status in conjunction with the interdisciplinary team
  • Conducting client assessments on an occasional basis Job Requirements To qualify for the Social Services Case Manager role you must enjoy working with clients and assisting people with PHYSICAL AND developmental disabilities.

Additional requirements of the Social Services Case Manager include:

  • Bachelor’s Degree in a field which offers content related to Social Work, Psychology, Counseling, Special Education, and/or Nursing
  • Master’s Degree preferred
  • 1-2 years’ experience working with HOME HEALTH CLIENTELE
  • Bilingual language ability (Spanish to English) is highly desirable!

All employment at PALLADIA HEALTH requires face-to-face, in-person interactions with PALLADIA HELATH clients, family members, community, and co-workers. Candidates for employment must live within commute distance to their geographical location of their assigned clients. 100% remote work is available to ALL employees, a hybrid work arrangement with a blend of in-office and home-based work may ALSO be available for new employees after successfully demonstrating achievement of performance expectations following at least 3 months of FIELD work.

  • The practice offers competitive compensation based on experience. Medical benefits with a contribution is available to employees working a minimum of 32 hours per week. Work hours are subject to office hours and patient schedules with minimal overtime upon occasion. weekendS or on-call requirements. Additional benefits to be discussed upon hiring.
  • If selected for an interview, candidates will be asked to demonstrate technical skills. In addition, candidates will be asked to provide business references (beyond verification of employment). Employment offers are contingent upon meeting standard requirements for working in a medical practice such as successful background check, I-9 verification, TB testing and Hep B immunization (or waiver) and COVID19 vaccination (booster required UNLESS WAIVER APPLIES).
  • COVID-19 precautions We follow all CDC and state of California COVID-19 health protocols. All clients and employees are required to wear KN95 masks. All employees must be fully vaccinated (including booster). COVID19 Policies and Procedure Agreement to be signed on hiring date. Thorough and detailed sanitization for treatmentand equipment is enforced.
